MTM is medication therapy management.  MTM allows the pharmacist to do a thourogh overview of a patient's medications and disease states.  This allows the pharmacist to identify any gaps or duplications in a patient's current treatment.  This also allows the pharmacist to develop a current treatment plan to enhance patient understanding of medication therapy, ensure patient compliance, reduce patient costs, and prevent therapy complications.

The reason this will chage the future of pharmacy is because it is a billable service.  A pharmacist can execute an MTM and submit it for reimbursement from the patient's insurance.
